Enrollment Forms

Welcome!  We are very excited that your family will be joining our SPELC Preschool family!

We have gone electronic with our Enrollment Forms!  Now that you are enrolled, you will be receiving an email inviting you to join our new online enrollment website, Early Learning Ventures.  If you do not receive the Online Enrollment Invitation email please let us know as soon as possible.  Once in the ACS system you will be asked to assign a “security code”.  This security code will be used to access the Parent Portal and to sign your child(ren) in and out of SPELC.

ALL ACS ONLINE ENROLLMENT INFORMATION MUST BE COMPLETED PRIOR TO YOUR CHILD’S FIRST DAY IN THE PROGRAM, PER STATE LICENSING REQUIREMENTS.  ALL SIGNED MEDICAL FORMS AND MEDICATIONS, IF NEEDED,  MUST BE ON FILE IN THE SPELC OFFICE, AS WELL.

The ELC Office staff will be available for Health Status Record Form, Immunization Form and medication drop-off on Wednesday, August 1st (9am-12pm) or you may mail or email in your forms prior to end of July.

REQUIRED MEDICAL ENROLLMENT FORMS.  The following Health Status Record must be completed in full, signed by both you and your child’s Licensed Health Care Provider (where applicable) along with a Certificate of Immunization/Shot Record and returned to SPELC.

  • General Health Appraisal Form  (One for each enrolled child) – rev. 5/23/18
  • Certificate of Immunization/Shot Record needs to be printed for you by your physician’s office (One for each enrolled child)

DOES YOUR CHILD HAVE ALLERGIES OR A MEDICAL CONCERN?:

If your child requires any type of medication to be given while at school, please read the following carefully, download, print and complete the appropriate forms.

Allergies

Please notify the office if your child has allergies.  If your child has specific food allergies, please check with the ELC Office to review the list of ingredients in the daily snacks provided by SPELC.   If your child uses an EpiPen and/or an antihistamine such as Benadryl, SPELC requires an EpiPen & Antihistamine Health Care Plan Form, be completed and signed by Parent/Legal Guardian and a Licensed Health Care Provider.  If your child requires both an antihistamine and an EpiPen, only one form is required for these medications left at school.  All medications must be in their original container with the prescription label attached, when applicable. 

Medications

No medication will be dispensed by SPELC Staff, except for prescription medication to treat an on-going medical condition (such as, febrile seizures requiring medication for high fevers) where medication must be administered during program hours.  If any on-going medications need to be taken during program hours, SPELC requires an Authorization For The Administration of Medication Other Than EpiPen Form, be completed and signed by Parent/Legal Guardian and a Licensed Health Care Provider.  We will require one form for each medication left at school.  All medications must be in their original container with the prescription label attached, when applicable.

For the 2018-2019 School Year, all medications must be provided to us no later than August 1 in their original container with the prescription label attached.
